If you are a new patient to our office, please complete our online new patient forms before your arrival to our office. If you are unable to complete the forms online, please plan on coming in at least 20 minutes prior to your appointment to fill out the forms at the office. If you need help completing the forms online, please contact our office at 408-963-6120 during business hours and we will be more than happy to assist you.

100% Preventative Coverage:
2 preventative appointments, includes:
Doctor Exam (Once every six months)
Cleaning (Once every 6 months)
X-Rays (as needed)
Intra Oral Images (as needed)
Flouride for children 14 y/o or younger (Once every 6 months)
Upgradable to Perio Plan if Deep cleaning is necessary
20% Off all in-house restorative treatment necessary*
20% Off in-house teeth whitening**
Periodontal Plan1
100% Preventative Coverage:
4 preventative appointments per year, includes:
Doctor Exam (Once every six months)
Scaling & Root Planing (SRP) (up to four quads if deemed necessary)
Perio Maintenance (once every 3 months)
X-Rays (as needed)
Intra Oral Images (as needed)
25% off all in-house restorative treatment necessary *
20% off in-house teeth whitening treatments.**
50% off Oral-B Tooth electronic tooth brush. Limit one per membership.**
40% off Perio Protect Treatment
*All basic and major restorative treatments performed at OCDC: includes crowns, core build up, fillings, denture fabrication and repair, night guards, root canal therapy, and extractions. Some procedures need to be handled by a specialist. Discount does not apply to any work referred out to any specialist. Additionally, discounts cannot be applied to Invisalign or Perio Protect Treatments.
** Discounts cannot be combined with any other promotion or discounts.

1 All eligible treatments must be started within 1 year of enrollment date. No carry over of any eligible benefits.
Standard office policies apply to these membership programs: Late Cancellation / No Show Fee of $75 per appointment. All procedures require payment in full at time of service.
